Virtual Learning
If you are taking a virtual class, it is the STUDENT'S RESPONSIBILITY to register for the course on the requested platform (Fulton Virtual or Georgia Virtual). Failure to do so will result in the loss of the virtual courses for the school year and you will be placed in face-to-face courses.
Certificate of Enrollment & ADAP Replacement (for DDS)
Division of Driver Services (DDS) Proof of Enrollment
The DDS now accepts the following documents for Proof of School Enrollment:
- Unofficial Transcript - found under "Documents" in the Infinite Campus Student or Parent Portal.
- Current School Identification Card - a copy must be made and submitted
- Notarized Certificate of School Enrollment Form - A notarized Certificate of School Enrollment Form for either a Learner's Permit or License can be requested by visiting the AHS Front Office.

ADAP Replacement
ADAP certificates are issued as part of a student's Health class. ADAP replacement certificates are available.

If you cannot find your certificate using the link above or you took your Health course through Fulton Virtual or Georgia Virtual School, please send an email to eADAPsupport@dds.ga.gov to request a copy of your certificate. Include your phone number, full name, birth date, and email address.

Medication
Student Health Services provides medical authorization forms to:
- Allow medication to be administered at school (SHS-1)
- Allow students to carry medications at school (SHS-2)

Meningococcal Vaccine Requirement
11th Grade Meningococcal Conjugate Vaccine (MCV4) Booster
Due September 5, 2025.
For the 2025-2026 school year, all 11th-grade students must have an updated GA 3231 immunization certificate on file in order to remain active at Alpharetta High School. Effective July 1, 2021, children 16 years of age and older, who are entering the 11th grade (including new entrants), must have received one booster dose of the meningococcal conjugate vaccine (MCV4) unless their initial dose was administered on or after their 16th birthday.
The Georgia Department of Public Health has developed a new Affidavit of Religious Objection to Immunization Form 2208 for use by all new students enrolling in a Fulton County School beginning with the 2015-2016 school year who wish to submit a religious exemption. The form can be found at the Georgia Department of Public Health under forms and brochures.
GA 3231 immunization forms can be emailed to AHS Registrar, Donna Byrd, or dropped off in the front office.
FCS District Health Services
2025-2026 Back-to-School Immunization Requirements:
- Rising 7th Grade Students,
- New Entrants/Students Grades 8 - 12th and
- 11th Grade Students
- Due September 5, 2025.
11th Grade: All students entering or transferring into the 11th grade will need proof of a meningococcal booster (MCV4) unless their first dose was received on or after their 16th birthday. Meningococcal disease is a serious bacterial illness that affects the brain and the spinal cord. Meningitis can cause shock, coma, and death within hours of the first symptoms. To help protect your children and others from meningitis, Georgia law requires students to be vaccinated against this disease unless the child has an exemption.

Personal Fitness Waiver
Residency Verification Form
In accordance with the Fulton County Board policy, all 9th and 10th graders, new enrollees, or households with a change of address must submit proof of residency. To submit proof of residency:
- Gather two proofs of residency as outlined below.
- Utility Proof (only one is needed)
- Electric Monthly Statement
- Water Monthly Statement
- NOTE: Gas bills are not accepted.
- Residence Proof (only one is needed)
- Home Mortgage Bill
- Home Sale Contract
- Current Apartment/House Lease
- Section 8/HUD Housing Document
- Current HOA Bill
- Utility Proof (only one is needed)
- Please only send acceptable documents outlined by Fulton County. Do not send documentation without completing the Residency Verification Microsoft Form below.
Due to the sensitive nature of this process, the Microsoft Form below can only be accessed using the student's FCSTU credentials.

Failure to submit the required form and supporting documentation will result in the student being administratively withdrawn from AHS.

If you uploaded your residency documents while completing the FCS census update process in Infinite Campus, please email camhsresidency@fultonschools.org to let us know. We will check Infinite Campus and let you know if we need anything additional.

Student Enrollment/Withdrawal Information

New Student Registration
- Gather Required Documents - All documents must be submitted prior to pre-registration appointments.
- Proof of Residency - View Accepted Items HERE!
- Eye, Ear, Dental, and Nutrition Screening (GA FORM 3300) must be signed by the health department or a licensed Georgia Physician.
- Immunization Certificate - Georgia law requires students to be immunized against measles, mumps, polio, rubella, whooping cough, tetanus, diphtheria, hepatitis B, haemophilus influenza, varicella and meningitis or provide a notarized waiver for medical or religious exemption. Immunization information must be on the proper Georgia Department of Human Resources form; these are available through the Fulton County Department of Health and Wellness or local physicians (Form 3231).
- Proof of Date of Birth - A certified copy of a birth certificate, certified hospital issued birth record or birth certificate, military ID, valid driver's license, passport, or an adoption record.
- Social Security Card - Social security numbers are voluntary. However, the lack of a Social Security number will result in the denial of the HOPE scholarship. Students who enroll without a Social Security card (for example, international students, those who sign a 30-day waiver, etc.) will be issued a number by the school system, beginning with the numbers 999-.
- Academic Records - Please bring the most recent report card, withdrawal form from previous school and/or transcripts. These are essential for proper placement of students. Students in grades 7-12 also will need copies of any discipline records.
- International Academic Student Records - Students that are coming from out of the country, please contact the AHS Registrar for instructions.
- 504/IEP/ESL – If your student has a 504 plan, receives Special Education services or receives or have received English as a Second Language services please let us know and forward those records to us before your appointment.
- Complete Online Enrollment Form and upload required documents. For more information, please visit the FCS Registration Homepage.
- Email Donna Byrd (byrdd@fultonschools.org) for next steps with the completed online application number and for next steps.
- Once the Registrar reviews the online application and approves, the prospective parent will receive a "Pre-enrollment" email with a link to make a conference call appointment with the student's assigned counselor.
- Once the assigned counselor completes the pre-enrollment conference, she will inform the Registrar if the student is approved to start. The Registrar enrolls students and sends a "Congratulations" email with all welcoming AHS information.

- Bring the completed withdrawal form and all school issued supplies when you arrive for your appointment.

Refund and Balance Transfer
Students Graduating or Leaving the District
Students who are graduating or leaving the districts can request a refund from their school nutrition account. To make a request, download and complete the refund request form at the link below. The form should be emailed to the AHS Nutrition Manager, Luci Araujo, at araujola@fultonschools.org.
Transferring Balance
Balances can be transferred to another FCS student. Contact the Cafeteria Manager, Luci Araujo, at araujola@fultonschools.org.

Updating Parent/Guardian and Student Information
Updating Email and Phone Numbers
To ensure your household receives emails from Fulton County Schools and Cambridge High School, please ensure that all guardians have the correct email address in Infinite Campus. It is also important that guardians can be reached during an emergency or if the school needs to contact you by ensuring that home, work, and cell phone information is correct. These updates are in Infinite Campus, under "More", then "Family Information."

Student Registration Information Changes
When your student's registration information (Census Data) needs to be updated, including:
- Primary household address
- Parent or guardian information
- Emergency contact information, or
- Student information (medical or new student)
These changes are made in Infinite Campus, under "More," then "Online Registration." Note that address changes must be verified through the Residency Verification Process. Work Permit
Request a Work Permit
The process for issuing work permits is a multi-step process through the Georgia Department of Labor.
- The student completes the minor portion of the online form, found at the link below, and provides his or her employer with the "MSK".
- Once the employer completes their part through the GDOL online form, the employer will give the student a printed confirmation to give to the school.
- The student should then bring the MSK# to Mary Franchak in room 1330, along with a copy of their birth certificate if we don’t have one on file, so that the school can access and provide the completed permit.
